When You First Arrive
When you arrive at our office, you will be asked to complete a Chiropractic Case History & Patient Information Form about your reason for seeking Chiropractic care and your past health history.

| If you are a new patient under the age of 18 it will be necessary for you to have a parent complete and sign a Consent to Treatment form. |

| In some cases after your first visit, it may be helpful for Dr. Spencer to review your health history by requesting the medical records from those health professionals under which you have previously received care. |

Insurance and Payment
The Chiropractic care provided by Spencer Chiropractic, LLC is covered by many different inurance plans, although the type and amount of coverage depends on your individual policy. For this reason, we ask that you bring your insurance card at the time of your first visit. Our office will be happy to contact your insurance company to find out what your specific coverage is, your yearly deductible if there is one, and whether that deductible has been met. If your insurance covers chiropractic care, we will file your insurance forms for you. In many cases, you may be responsible for a deductible and a portion of the fee not covered by your contract. |

Your First Visit
Your first visit will take approximately 45 minutes to 1 hour. During the first portion of your visit will consist of a review of your current and past health history, including your reason for seeking chiropractic care. Dr. Spencer will consult with you and answer any questions you may have regarding the Chiropractic care you will receive at Spencer Chiropractic, LLC. Please feel free to speak openly with Dr. Spencer.
The next step is the examination, which will consist of a thorough spinal and nervous system review that includes palpation of the spine. With the important information gathered during your examination, Dr. Spencer can make a more accurate determination of the type of Chiropractic care you will need as well as the initial frequency of this care. The results of your examination are explained to you during your second visit, the Report of Findings, which is also when most people will receive their first adjustment.
Some Chiropractors take x-rays of each one of their new patients. Dr. Spencer does not feel this is always necessary. Ocassionally however, X-rays may be taken depending on your situation. Dr. Spencer will not take x-rays of certain patients, including; pregnancy, most children or if you have recent usable x-rays of the area in question. Aside from these occasions, x-rays are a common part of the examination process when looking for subluxations and making a diagnosis. X-rays provide important information, including the specific position of the subluxated bone, the quality of the spine, and any possible contraindications to adjusting areas. The dosage level of the x-rays is lower than that used in a dental x-ray as well as daily sun exposure for one year. |

Your Second Visit
Your second, most important visit to this office is your Report of Findings which takes approximately 30 minutes. During this visit, Dr. Spencer will explain the results of your initial examination and x-rays if they were taken. You will be given a care plan that addresses your specific needs and is designed to give you the best and fastest results possible. We want you to be actively involvement in developing a care plan for you and will make recommendations of things you can do yourself between visits to help you reach your goals more quickly. It is important that you be on time for this visit. |
